The responsibility of the football club for the above player will commence from the moment he / she arrives at the training location and makes themselves known to the coach / manager and ceases when he / she leaves the training area after the session. We undertake as members of Claydon Football Club to at all times conduct our behaviour in accordance with and abide by the rules and regulations of the Claydon Football Club constitution and codes of conduct as laid out by the F.A. Please note the following in relation to club equipment and policies 1. All equipment provided by the club to players in order to participate "the kit" remains the property of the club and must be returned when requested or when the above player leaves the club 2. Any and all fines levied by the F.A against the above player, regardless of whether the club has paid the fine already, will be paid for by the fined player or their parents and where the club has already paid the fine will be reimbursed within 30 days of the fine being recieved. Non payment of fines will result in the above player being inelligable for selection. Please check the box below to confirm you have read this statement and understand it.

Statement for permission. By ticking this box I am giving permission for my Son/Daughter to continue to train with Claydon FC during these current circumstances following the FA’s guidelines surrounding Covid-19. I am aware there is a strict risk assessment in place that all team managers/coaches and appropriate adults are adhering to and should I request a copy of the risk assessment in place that this would be made available for viewing. I will ensure that if my child becomes symptomatic I would not send him/her to training and will notify the relevant team manager of this immediately.

Half Seasons Subs For U8 - U18 Players (£60)

Under 7's Full Seasons Subs (£35)

Training only (£35)

Youth disability football (£30)

Adult disability football (FREE)

Payment can be made for either the full season (£120), or half a season (£60). If paying for a half season, the first payment needs to be made upon registration, with the remaining payment made by the end of January 2024. (This does not include U7). If paying for a full season, the payment is preferred upon registration.

Payment is preferred by BACS, but cheque or cash payments can also be taken (please make cheque payments to Claydon FC). If you are paying by BACS or cheque, please submit the form and make your payment after submission.

If paying by BACS, please state the last name of the player, age group and name - e.g. CLARKE U17 DRAG.
